Children's Pajamas Recalled by Bella Bliss Due to Violation of Federal Flammability Standard

Units: About 2,300

Hazard: The pajamas fail to meet the federal flammability standards for children's sleepwear posing a risk of burn injury to children.

Description: The pajamas are a two-piece, cotton sleepwear set sold in sizes 2 to 12. The pajamas are white, blue, pink or red. "Bella Bliss" is written on the neck tag.

Sold at: Bella Bliss catalog and website, and specialty clothing retailers and online stores from January 2008 to June 2011 for about $48 to $58.

Remedy: Children should stop wearing the recalled sleepwear immediately and consumers should return it to the retailer where the product was purchased for a refund, exchange or store credit.

Collective Brands Recalls KEDS Girls' Shoes Due to Laceration Hazard

Name of Product: KEDS® "Know It All" Girls' Shoe

Units: About 45,000

Hazard: Ornamental stars on the heel of the shoe may loosen, posing a laceration hazard.

Incidents/Injuries: The firm has received 27 reports of cuts and scratches resulting from metal stars that loosened from the heel of the shoe.

Description: This recall involves KEDS® girls' rubber soled shoes. The shoes are black and pink with white trim and a pink loop on the heel. "KEDS" appears on the tongue and heel of the shoe. The style number KY40098A is printed on the underside of the tongue. The shoes were sold in girls' sizes 12 to 5.

Sold at: Various department stores and online retailers from June through October 2011 for about $23.

Remedy: Consumers should take these shoes away from children immediately and contact Collective Brands to receive a gift card for $30 redeemable at Stride Rite stores or striderite.com.

Children's Animal Slippers Recalled Due to Choking Hazard; Sold Exclusively at Family Dollar Stores

Hazard: The animal's eyes can detach from the slippers, posing a choking hazard to young children.

Description: The recalled children's slippers have brown or tan dog faces, yellow duck faces and tan lion faces. The name "Kidgets" and the size appear inside the slippers on the soles. "FD9619108020690611" is printed inside the slippers on the side.

Sold exclusively at: Family Dollar stores nationwide from September 2011 through October 2011 for about $5.

Remedy: Consumers should immediately take the slippers away from children and return them to any Family Dollar store for a full refund.

Boy Scouts of America Recalls Cub Scout Wind Tech Jackets Due to Strangulation Hazard

Units: About 5,400

Hazard: The jackets have retractable cords with toggles at the hood/neck area and at the waist, which can pose a strangulation or entrapment hazard to children. In February 1996, CPSC issued guidelines (pdf) which were incorporated into an industry voluntary standard in 1997, to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets and sweatshirts.

Description: This recall includes the blue Cub Scout Wind Tech jacket sold in youth sizes. The jackets are nylon with a polyester lining, long-sleeve, with a full zipper front and a Cub Scout wolf head emblem embroidered on the upper left chest. SKU numbers 73291, 73292, and 73293 are printed on the hangtag that is attached to the jacket at retail.

Sold at: Boy Scouts of America retail outlets nationwide and online at www.scoutstuff.org from November 2009 through July 2011 for approximately $32.

Manufactured in: China

Remedy: Consumers should immediately remove the cords from these jackets to eliminate the hazard, or return the jacket to Boy Scouts of America for a full refund.

CooperVision Continues the Voluntary Recall of Avaira™ Toric Contact Lenses

ContactsIn full cooperation with the United States Food and Drug Administration, CooperVision, Inc. is continuing to work with the FDA, distributors, customers, eye care practitioners and lens wearers to complete the worldwide recall initiated on August 19, 2011 on certain lots of Avaira™ Toric contact lenses.

If you wear Avaira Toric contact lenses and experience any symptoms, CooperVision recommends that you stop wearing the lenses immediately.  Contact your eye care practitioner for advice.

If you wear Avaira Toric contact lenses and are not experiencing symptoms, CooperVision recommends that you:  

Go to the CooperVision recall web page at www.coopervision.com/recall,  and enter the lot number of your package to determine whether your lenses have been recalled.

  • If your lenses have been recalled, immediately remove your existing lenses, discontinue lens wear; and return your lenses to your eye care practitioner or point of purchase. 

The recall was initiated because of the unintended presence of a residue on certain lots of Avaira Toric lenses, which may cause hazy vision, discomfort, or eye injuries requiring medical treatment. Not everyone experiences the same symptoms. Since the initiation of the recall, the company has received some additional complaints of severe eye pain. Avaira Toric contact lens wearers that experience any symptoms should stop wearing the lenses immediately and speak with their eye care practitioner.

To date, the recall has impacted approximately 600,000 Avaira Toric contact lenses in the U.S.

C. O. Lynch Enterprises Inc. Recalls Itasca Fusion Hiker Boots Due to Foot Injury Hazard

Units: About 5,200

Hazard: The boots could fail to provide the intended protection against compression and impact, posing the risk of a foot injury to consumers. The boots have hard composite toe material rather than steel.

Description: Itasca men's shoes sizes 7 to 14 with the model name Fusion Hikers are being recalled. The model name Fusion, style number 45513 and order numbers 22215, 22216 or 22217 are printed on a label on the underside of the tongue. The word Itasca and the Itasca logo are embossed on the top of the tongue of the shoe and are also imprinted on the removable insole. The shoe is black with leather and nylon upper and a thermoplastic rubber (TPR) outsole. "Steel Toe" is printed on a tag on the outside of the shoe.

Sold exclusively at: Big 5 Sporting Goods Stores between March 2011 and June 2011 for about $50.

Remedy: Consumers should stop wearing the recalled boots immediately and return the shoes to C. O. Lynch for a full refund.

Hazard: The garments fail to meet the federal flammability standard for wearing apparel, posing a fire hazard to consumers.

Description: The scarves, dresses, skirts and blouses are made of 100 percent silk and come in a variety of designs. Nearly all items were returned to Chanel in May 2010.

Sold at: Chanel Boutiques, Neiman Marcus and Maxfield stores for $430 to $3,650 in March and April 2010

Remedy: Consumers should immediately stop using the garments and contact the firm to receive a full refund.

Hazard: The garments fail to meet the federal flammability standards for children’s sleepwear posing a risk of burn injury to children. The garments are being recalled because they do not meet the tight-fitting sizing requirements.

Description: This recall involves long sleeve boys’ and girls’ pajama sets that were part of the Enchanted Forest Collection, Forest Friends Collection or Golden Dragonfly Collection. The sets were sold in sizes 6 months to 4T in the following colors: cream and green; dark blue and light blue; and, pink and lilac.

Sold at: Small boutiques nationwide and online at the company’s website www.sagecreekorganics.com from August 2010 to February 2011 for approximately $35.

Description: The recalled infant girls sandals are white with decorative plastic flowers attached to the toes and sides. The sandals were sold under the style name "Aloma" in infant girls' sizes 2 through 5. "Circo" is printed on the inside and bottom of the shoe.

Sold exclusively at: Target stores and Target.com nationwide from January 2011 to May 2011 for about $10.

Remedy: Consumers should immediately stop using the sandals and return them to any Target store to receive a full refund.
